Updated Record of Divorce Form
The Department of Vital Records has revised the Record of Divorce Form to comply with the U.S. Supreme Court decision in the marriage equality case. In addition, the form is now a fillable form, which will be accepted by all courts.
The new form may be used beginning October 1. While the department encourages the use of the new form as of that date, it will continue to accept the old form until January 1, 2016. After that date, only the new form will be accepted. There will also be hard copies sent to the county clerks for distribution to attorneys in their area. However, even after the printed forms are available, attorneys are welcome to use paper copies of the form if they wish.
